What is Neuropathy?
Neuropathy, or peripheral neuropathy, is a disorder affecting the motor, sensory and autonomic nerves (otherwise known as the peripheral nerves) connecting our muscles, skin and internal organs to our spinal cord. While the exact cause of neuropathy is unknown, it is suspected to be for reasons similar to that of multiple sclerosis - a decline in the myelination of nerve cells and fibers.
In a normal person, each nerve cell produces a waxy lipid to wrap itself in called a "myelin sheath". This sheath allows the neurons to react quickly and precisely, ensuring the electrical charge of the nerve's message remains contained and ends up where it is meant to. In the case of peripheral neuropathy however, the specialized cells inside a nerve responsible for producing the myelin sheath degenerate and die off. Thus, the message carried by nerve cells is not so well contained.
When this occurs, rather than signaling one particular muscle, a neuron's electrical charge will leak and signal an entire region leading to an overstimulation, random impulses and impairment in the affected area. Though there are many types of neuropathy, this problem is at the root of each one.
It is the cause of this declining myelination that separates the major types of neuropathy.
- Pre-Diabetic/Diabetic Neuropathy. With nearly 60% of diabetics suffering nerve damage, it's not so surprising that, accounting for 30% of all neuropathy cases, pre-diabetic/diabetic neuropathy is one of the most common types of the condition. Peripheral neuropathy is more common in diabetics whose blood sugar levels are difficult to manage, are overweight, and over 40 years old.
- Compression Neuropathy. Sometimes referred to as "nerve compression syndrome" or "entrapment neuropathy", this type of neuropathy is caused by direct pressure on a single nerve and affects just one part of the body depending on the nerve. Compression neuropathy is most common among those with jobs requiring repetitive movements, but can occur when one frequently and for long periods of time, puts pressure on a certain area. For example, draping an arm over a chair or resting weight on their elbows against a table. The most common form of compression neuropathy is carpal tunnel syndrome - a condition which is becoming ever more prevalent as more people work jobs behind a keyboard.
- Inflammatory Neuropathy. Any infection, disease, or disorder can put you at risk for inflammatory type neuropathy. With inflammatory neuropathy, declining myelination is caused by viral or bacterial attack of nerve tissues. Risk factors include infectious diseases like Lyme disease, HIV/AIDS, or Hepatitis and autoimmune diseases like Celiac disease, Sarcoidosis, and Guillain-Barre Syndrome.
- Metabolic Neuropathy. As with inflammatory neuropathy, metabolic neuropathy occurs with a wide array of disorders, in this case metabolic. Risk factors include disorders like diabetes, mentioned above, as well as hypoglycemia, uremia and hypothyroidism. Metabolic neuropathy may also occur with kidney disorders, alcohol abuse and nutritional deficiencies. In this case, drop in myelination is caused by metabolic impairment.
- Hereditary Neuropathy. An extremely rare, yet under-diagnosed condition, the most common forms of hereditary neuropathy are Charcot-Marie-Tooth (CMT) Disease and Hereditary Neuropathy with Liability to Pressure Palsies (HNPP). In cases of hereditary neuropathy, genetic defects create a group of nerve disorders that lead to weakness and wasting of the muscles.
- Idiopathic Neuropathy. While in most cases of neuropathy the cause can be identified, in as many as 30% of sufferers, it cannot. For these people, neuropathy is labeled "idiopathic" - "peculiar, separate from" (idio) "suffering from a particular condition" (pathic). Idiopathic neuropathy typically occurs in people over age 60 and progresses very slowly - or not at all after initial appearance. However, this does not mean the condition is any less uncomfortable or disruptive to their lifestyle.
